SAP conducts evaluation of employee who has engaged in prohibited
conduct. 2.
SAP determines if employee needs assistance with the alcohol
misuse (or illegal drug) problem. The QSAP must provide appropriate
documentation direct to the DER. 3.
SAP refers for treatment and/or rehabilitation as appropriate. At
a minimum, the QSAP a Substance Abuse Education and
Awareness Program. 4.
The SAP cannot conduct
counseling, treatment, education or refer to affiliated facility.
5.
SAP must conduct follow-up
evaluation to determine if
employee has complied with recommendations for assistance. 6.
SAP will establish follow-up testing regimen. May recommend
drug/alcohol testing for those employees who have poly-substance abuse problems.
Must recommend at least six tests in the first year. May recommend continuation
of testing up to sixty months. The SAP may recommend "continuing
care" regimen (aftercare, supportive outpatient, 12-Step, etc). Letter
format to the DER 7. SAP is a licensed physician, Social Worker, Psychologist,
Certified Employee Assistance Professional (CEAP) or
a
drug and alcohol counselor certified by the National Association of Alcoholism
and Drug Abuse Counselors Certification Commission (NAADAC); or by the
International Certification Reciprocity Consortium/Alcohol and Other Drug Abuse
(ICRC); or by the National Board for Certified Counselors, Inc. and
Affiliates/Master Addictions Counselor (NBCC).
He must have knowledge and experience in diagnosis and
treatment of substance abuse disorders and must have knowledge of and be
familiar with these regulations and guidelines. Must successfully complete
qualification training and testing process as of John
H. Baxter meets the
qualification (credentials), and has completed the training and satisfactorily
